Positioning Health Leads to scale its model and provide leadership in addressing vulnerable families' resource needs as a standard part of care
This investment from the Robert Wood Johnson Foundation (RWJF) will support the first 30 months of implementation activities for Health Leads' 54-month strategic plan. The plan aims to catalyze fundamental shifts in the health care sector that help make it standard practice to integrate the meeting of patients' social resource needs into quality health care delivery. The total philanthropic capital needed for the 54-month plan is $33 million. Of the $16 million commitment from RWJF, $11 million consists of grant funds and $5 million will serve as a challenge grant contingent on Health Leads raising $11 million from other investors by September 1, 2015. At the end of RWJF's 30-month investment, Health Leads expects to have: (1) increased the number of its service desks at health care facilities from 21 to 38; (2) completed 4 evaluation studies; (3) added 8 health care partner institutions; and (4) secured 6 pilot contracts for advisory services.
